Before completing the three housing forms located below, new students are encouraged to obtain their Stetson ID# for security reasons. To find out how to obtain your Stetson ID# students need to go to the Banner Web Access Webpage.
If you are unable to obtain your Stetson ID # please notify the Registrars Office at 386-822-7140. If you are experiencing problems obtaining your Stetson ID# you can insert your social security number in any of the blocks on the housing forms that ask for your Stetson ID#.
To apply for housing at Stetson University each student must complete a Housing Agreement and Meningitis Vaccine form. We encourage that the student take their time and complete each form fully.

Form 2. Meningitis / Hepatitis B Form Waiver Form (Online Version)
** Florida law now requires individuals enrolled in an institution of higher education who reside in on-campus housing be vaccinated against Meningococcal Meningitis and Hepatitis B, or sign a waiver declining the vaccine.
After having read the enclosed information regarding the diseases and the availability and effectiveness of the vaccines, the individuals or parents of minors may decline the vaccinations by signing the waiver for each of these vaccines.

Form 3. Statement of Student Residency Documentation confirming that a student qualifies for an exemption from university housing requirement. The Residency Requirement Form is needed for any student choosing to live at any off campus residence. the form above must be filled out completely and notarized.
All new students are held responsible to all terms, policies, and procedures covered on all housing forms. Please refrain from entering multiple copies of any form. Thanks!

Form 4. Petition to cancel Residential Housing Agreement
Petition to Cancel Housing Agreement Form
This agreement shall be binding for the full academic year defined as Fall and Spring Semester during which the student is enrolled unless otherwise stated. This Agreement shall become effective on the first day of the Fall semester of the academic year stated above or the first day of the Spring semester for students initiating the agreement at that time, and shall terminate at the end of the ensuing Spring Semester. The student agrees to live in the designated University facility during the entire period of this agreement, or that portion of the Agreement, which remains after the student's enrollment.
Cancellations made after May 31 will be subject to a variable cancellation fee. Cancellations from JUNE 1- 30 will incur a $300 cancellation fee; Cancellations from JULY 1-31 will incur a $600 cancellation fee; Cancellations from AUGUST 1 TO THE START OF CLASSES will incur a $900 cancellation fee; And any cancellation fee after the start of the semester/academic year will incur a $1000 cancellation fee in addition to charges for the period of occupancy.
