Every person who'd like to use the Hollis Center's facilities and/or attend on-campus events must complete our electronic General Use Wellness and Recreation Liability Waiver and Informed Consent

You must be at least 18 years old to sign these forms. If you're not yet 18, you will still sign this form but a copy will be emailed to your parent/guardian and they must sign the electronic form and submit it as well.

Monday through Thursday
7 a.m. - 10 p.m.

Friday
7 a.m. - 8 p.m.

Saturdays and Sundays
1 p.m. - 5 p.m.
